This form must be completed and returned to the church office 7-10 days before any activity or meeting is scheduled in the building. All requests are on a first come first serve basis.

*If Pastor’s service is needed for a wedding, he requires 6 months notice for pre-marital counseling.
*If an emergency meeting is needed, members must notify Church Office within 24 hours in writing (no exceptions).
**DEPENDING UPON THE ACTIVITY, IF THE EVENT REQUIRES MRS. BARBARA HOLLIMAN SERVICES THE FEES WILL BE NEGOTIATED IN ADVANCE**
